اور حلفیہ بیان کرتا /کرتی ہوں کہ:
1۔ میرے پاس اپنا ذاتی ٹریکٹر (کم از کم 50 ہارس پاور) زرعی آلات و مشینری کو چلانے کیلئے قابل استعمال حالت میں موجود ہے ۔
2۔ میں منصوبہ کی مدت /تین سال کے دوران سکرونتی کمیٹی کی طرف سے دیئے گئے ٹارگٹ کو حاصل کرنے کا پابند ہوںگا /گی۔
3۔ میں نے کسی دوسرے ضلع /تحصیل میں اس سکیم کے تحت مالی اعانت کے لیئے درخواست نہیں دی۔
4۔ میں نے اس سے پہلے یہ سہولت کسی وفاقی /صوبائی ادارے سے حاصل نہیں کی ۔
5۔ میری پورے پنجاب کے نہری علاقہ میں ذاتی ملکیت زرعی اراضی کہیں بھی پچیس (25) ایکڑ سے زائد نہ ہے ۔
6۔ میں محکمہ مال یا کسی دوسرے مالیاتی ادارے کا /کی نادہندہ نہ ہوں ۔
7۔ میں مہیا کردہ زرعی آلات و مشینری کو ذاتی استعمال کے علاوہ کرایہ پر سہولت کیلئے استعمال کروںگا/گی۔
8۔ حکومتی شرائط کے مطابق اگر مالی اعانت پر خرید کیلئے قرعہ اندازی میں میرا نام نکل آیا تو میں8دن کے اندر حکومت پنجاب کے منظور شدہ زرعی مشینری و آلات کے تیار کنندہ یا اس کے مجاز ڈیلر کے ذریعہ زرعی مشینری و آلات بک کراﺅں گا /گی۔ اگر کسی بھی طور پرمبینہ حادثہ کے دوران زرعی مشینری و آلات بک نہ کروا سکا /سکی تو میری مالی اعانت

9۔ حکومتی شرائط کے مطابق اگر مالی اعانت پر خرید کیلئے قرعہ اندازی میں نام نکلنے کے بعد حادثاتی طور پر مسمی/مسمات کے انتقال کی صورت میں زرعی مشینری و آلات کے حصول کے لیئے اُمیدوار کانام ۔۔۔۔۔۔۔۔۔۔۔۔۔نامزد کرتا /کرتی ہوں جس کا شناختی کارڈ نمبر ۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔اور تصدیق کے لیئے فوٹو کاپی لف ہے ۔
10۔ میں اس سکیم کے تحت حاصل کردہ زرعی مشینری و آلات عرصہ 3 سال تک کسی انداز میں بھی کسی دوسرے شخص کو فروخت نہیں کروں گا/گی اور نہ ہی قبضہ منتقل کروں گا /گی۔
11۔ الاٹمنٹ کے بارے میں چیئرمین الاٹمنٹ کمیٹی کا فیصلہ حتمی ہوگااور اسے کسی بھی عدالت میں چیلنج نہیں کروں گا/گی۔
12۔ ایک درخواست ایک مشین /زرعی آلہ کیلئے کارآمد ہوگی۔ ایک درخواست دہندہ ایک وقت میں ایک ہی طرح کی ایک مشین /زرعی آلہ کا اہل ہوگا دوسری قسم کی مشین /زرعی آلہ کیلئے علیحدہ درخواست ہوگی۔
میرے قومی شناختی کارڈ نمبر ۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔۔کی تصدیق شدہ کاپی لف ہے ۔ نیز میں اقرار کرتا /کرتی ہوں کہ مندرجہ بالا شرائط میں سے کسی ایک شرط کی خلاف ورزی یا غلط بیانی کی صورت میں بھی حکومت پنجاب کو مالی اعانت کی کل رقم نقد واپس کرے گا/گی جو کہ مجھ سے بطور بقایا جات زرعی اراضی قابل وصول ہوگی۔
گذارش ہے کہ میری درخواست تابع مندرجہ بالا شرائط منظور فرمائی جائے اور قرعہ اندازی میں شامل کی جائے ۔

And I declare under oath that:
1. I have my personal tractor (minimum 50 horse power) in usable condition for driving agricultural implements and machinery.
2. I will be committed to achieve the target given by the Scrutiny Committee during the plan period/three years.
3. I have not applied for financial assistance under this scheme in any other district/tehsil.
4. I have not received this facility from any federal/provincial institution before.
5. My personal agricultural land is not more than twenty-five (25) acres anywhere in the canal region of Punjab.
6. I am not in default of finance department or any other financial institution.
7. I will use the agricultural equipment and machinery provided for convenience other than personal use.
8. According to the government conditions, if my name comes out in the draw for the purchase of financial assistance, then I will book the agricultural machinery and equipment through the manufacturer of agricultural machinery and equipment approved by the Punjab government or its authorized dealer within 8 days. If in any way the agricultural machinery and equipment could not be booked during the alleged accident, then my financial assistance
It will be an illusion.
9. According to the government conditions, if the names of the candidates for the purchase of agricultural machinery and equipment are accidentally passed away after the name is drawn in the lottery for the purchase of financial assistance. I am the nominee whose identity card number is... and a photocopy for verification.
10. I will not sell or transfer the possession of agricultural machinery and equipment acquired under this scheme to any other person for a period of 3 years.
11. The decision of Chairman Allotment Committee regarding allotment shall be final and shall not be challenged in any court.
12. One application will be applicable for one machine/agricultural implement. An applicant shall be eligible for one machine/agricultural implement of the same type at a time, separate application shall be made for other type of machine/agricultural implement.
Certified copy of my National Identity Card No. Also, I agree that in case of violation or misrepresentation of any of the above conditions, I will refund the total amount of financial assistance to the Government of Punjab in cash, which is due to me as arrears of agricultural land. Will be received.
It is requested that my application be approved subject to the above conditions and included in the draw.
